In this model, a carefully vetted Home Care Provider welcomes the individual into their home and daily life, supporting them as an active member of their family and social network.
Through this supportive home environment, individuals with developmental disabilities can build life skills, develop self-confidence, and participate more fully in their communities. Our approach emphasizes connection, inclusion, and the development of lasting relationships that help create a fulfilling and meaningful life.
Home Care Providers play an important role in the individual’s support team. They participate in person-centered planning meetings and conferences, working closely with family members and program staff to help achieve the goals outlined in each individual’s service plan.
Every Home Care Provider goes through a thorough screening and matching process. This includes interviews with the individual, their guardian, and the program manager to ensure the best possible compatibility and supportive living environment.
All Home Care Providers receive comprehensive training and continuous guidance from program staff and an Agency Nurse Trainer. This ensures they are fully prepared to support each individual’s unique needs while meeting all state and federal care standards.
